Survival game that mostly use guns, aiming is not on Dev priority to improve.
Manual aim only for 1st slot only. If you start with non gun item, no more aiming until you sell the 1st slot item to move up the gun item. There's no inventory organizing to help with this problem either.
This have been in the game since demo and post 1.0 release.
