A lot of people are rating this low purely because the content is so extreme. This is very much not an HR approved porno, there is plenty of rape, murder, despair, engorged magical clitorises, fleshlight nipples, vaginal prolapses, inhuman pregnancies, and all manner of nastiness in this adventure of vengeance.
I wouldn't call it groundbreaking, but in terms of how depraved it is, it is quite refreshing. The gal you play as goes through a very tough time, and is forced into nudity or near-nudity right from the outset, and not long after is forced to declare her servitude to every man around her.
The gameplay is pretty standard, but I appreciated making her cartoonishly oversized clitoris the focal point of the protagonist's power. This game is shameless in bending its world around its fetishes, which is something I always appreciate in porn. If you feel inclined to experience something especially mean spirited, you could do a lot worse.
